Understanding Disability: Types, Characteristics, and Support

障害の理解 · Updated 30 October 2026
The 障害の理解 field covers physical, intellectual, developmental, and mental disabilities together with their support systems. The modern view — formulated in the ICF and the social model — is that barriers in life come not only from the condition of the body, but also from the environment and society’s attitudes. Questions in this field often ask about the distinctive characteristics of each condition and about pairs of terms that are easily confused.

The concept of disability and the social model
The old model (医学モデル) viewed disability purely as a medical problem within the individual. The social model (社会モデル) sees barriers as arising because the environment and society’s systems are not yet accommodating — stairs without elevators, information without text, prejudice. The ICF combines the two: body functions, activities, and participation are influenced by environmental factors.
The legal umbrella is the 障害者基本法, and welfare services are governed by the 障害者総合支援法. There are three official handbooks: 身体障害者手帳 for physical disabilities, 療育手帳 for intellectual disabilities, and 精神障害者保健福祉手帳 for mental disabilities.
- 社会モデル: barriers also arise from the environment and society
- Three handbooks: physical, intellectual (療育), mental
- 障害者総合支援法 governs disability welfare services
The main physical disabilities
Spinal cord injury (脊髄損傷) causes paralysis according to the level of the injury: a neck injury (頸髄) causes paralysis of all four limbs (四肢麻痺), while an injury below that causes paralysis of both legs (対麻痺). Both carry risks of elimination problems and pressure ulcers, so skin care and a toileting schedule are very important.
Other conditions that often appear: cerebral palsy (脳性麻痺), muscular dystrophy (筋ジストロフィー) which progressively weakens the muscles, ALS, which attacks the motor nerves while consciousness stays clear, and internal disabilities (内部障害) such as heart and bladder disorders — invisible from the outside and marked with the ヘルプマーク (Help Mark) so that people around them give consideration.
- 頸髄損傷 → 四肢麻痺; below the neck → 対麻痺
- ALS: the motor nerves are damaged, consciousness and sensation remain
- ヘルプマーク: a marker for disabilities that cannot be seen
Intellectual and developmental disabilities
Intellectual disability (知的障害) is characterized by difficulty with abstract thinking and understanding complex matters; effective support uses short, concrete sentences, pictures and visual schedules, and the same explanations repeated without rushing.
Developmental disorders (発達障害) include the autism spectrum (自閉スペクトラム症) — strong on patterns and routines, sensitive to sudden change; ADHD — difficulty concentrating and a tendency to be impulsive; and specific learning disorders (学習障害). A predictable environment helps far more than repeated scolding.
- 知的障害: support with concrete language and visual aids
- Autism: announce schedule changes in advance
- 発達障害 is governed by the 発達障害者支援法
Mental disability and life support
Schizophrenia (統合失調症) has positive symptoms — auditory hallucinations and delusions that appear in the acute phase — and negative symptoms such as flat affect, withdrawal, and loss of motivation that persist in the calm phase. Medication is the foundation; care workers support regular medication-taking and daily routines without forcing things when symptoms worsen.
Support for people with mental disabilities is directed toward life in the community: group homes (グループホーム), employment support (就労支援), and social activities. Peer support (ピアサポート) from people with the same lived experience, and support for families, are important parts of this, because stigma remains the biggest barrier to participation.
- 統合失調症: positive symptoms (acute) vs negative symptoms (calm)
- Direction of support: living and working in the community
- ピアサポート and family support fight stigma

Frequently tested points
- A pair that is often tested: 頸髄損傷 = 四肢麻痺, and a lower injury = 対麻痺.
- The ヘルプマーク is for disabilities that are not visible, such as 内部障害 and early pregnancy.
- For the autism spectrum, sudden changes are a source of anxiety — give notice first.
